If you do not have a computer, paper copies of this form will be available at both Kinmount pharmacies and can be dropped in the mail slot outside the KDHC.

Once again we ask the greater Kinmount community to get some sponsors, walk awhile, and help us raise funds to continue the recruitment process of bringing an additional doctor to our Health Centre.
